Colorectal cancer , a leading cause of cancer mortality, has shown increasing incidence among older adults, yet the benefit of oxaliplatin-based adjuvant chemotherapy in this group remains unclear. To address this gap, a study evaluated whether an optimal age threshold exists for survival benefit from adding oxaliplatin to fluoropyrimidine-based therapy, involving 53,147 patients. The study found that oxaliplatin was not associated with improved survival in stage II colorectal cancer across all age thresholds.

In stage III disease, oxaliplatin significantly improved survival up to age 70 years (adjusted HRs (AHRs)=0.59), with 5-year overall survival of 84.8% vs 78.1% without oxaliplatin.
