Primary Myelofibrosis (PMF), previously known as chronic idiopathic myelofibrosis (CIMF) or agnogenic myeloid metaplasia (AMM), is one of the chronic myeloproliferative disorders characterized by clonal proliferation of myeloid cells with variable morphologic maturity and hematopoietic efficiency. This article focuses on clinical manifestations and diagnosis of PMF. Primary myelofibrosis is the least frequent among the chronic myeloproliferative disorders with an estimated incidence of 1.5 per 100,000 per year. PMF is mainly seen in middle aged and elderly patients.

Approximately 5% patients are diagnosed at the age of 40 years and 17% patients at 50 years. The median age presentation is 67 years.
