Case presentation: A 54-year-old man was experiencing binocular vertical diplopia for the past 4 months. His symptoms seem progressively worsening over time. During last week, he noticed the vision in the right eye was starting to darken. During these four-months, the patient was being treated for his sinus disease and nosebleeds (which were present for the past 10 months). Treatment: A polypectomy/anstrostomy was performed and on post-operative day 1, his diplopia was still noticed, although his nosebleeds and sinus disease had improved.
After one week of the surgery, there was bulging in left eye accompanied by a left facial droop as well. Initially, Bell’s palsy was suspected and treatment was started accordingly but his symptoms did not improve. Examination: External image of both the eyes and fundus photographs are attached.
