A 56-year-old farmer presented with gradual onset of tightening chest pain on his left side. Over 6 hours, the pain reached its maximum intensity.
Patient history: A manual worker Non-smoker and occasional drinker No history of gastrointestinal or upper respiratory tract illnesses Chest pain condition: Radiated from back to the front of the chest No radiation to arm or neck Associated with sweating Physiological examinations: Pulse rate :42 beats/minute Blood pressure: 130/80 mmHg No murmur & no signs of acute left ventricular failure Normal neurological examination No skin rash or eschar ECG showed complete heart block with a complete absence of AV conduction and ventricular escape rate of 45 beats/minute Treatment Considering acute ischemic events, aspirin, clopidogrel, and statin were given, but subsequent ECG revealed no dynamic change.
