Case presentation A 68-year-old man presented to a new endocrinologist for a regular checkup. He complained of decreased libido, erectile dysfunction with response to sildenafil, and weight gain during the past 2 to 3 years. He entered puberty at around 13 years of age and denied changes in body hair or gynecomastia. At the age of 58 years, he was diagnosed with idiopathic secondary hypogonadism and was treated with testosterone injections for 7 years.
