CASE SUMMARY The 2-year-old male child was admitted to the emergency department because of tractor crash accident 5 weeks earlier. Case history The patient fell from a moving tractor and the back wheel of the tractor had passed over the child’s thorax. A chest X-ray performed previously, 24 hours after the accident had been normal (Figure 1). The child presented acute respiratory distress 10 days ago and was initially wrongly diagnosed with pneumothorax in a rural hospital and an intercostal drain was inserted and intravenous fluids and antibiotics administered.

The drain was removed after three days as no air or liquid was drained. Physical Exam At the emergency department, he presented sudden onset of dyspnoea, floppiness, and pallor, although no serious injury was found.
