A 52-year-old woman presented with radicular pain corresponding to L4–L5 segment. History The patient had fibromyalgia and depression. She had a medication history of gabapentin, dextropropoxyphene, and pantoprazole. The patient was allergic to penicillin.

She had a smoking history of 1-2 cigarettes per day for about four years. Examination Gait difficulties were observed on heels and toes.
