Present illness: A 52-year-old  female was complaining of weakness and fatigue, although she continued to work. She attributed these changes to menopause. Her menstrual periods have been irregular but occasionally heavy. In past 6 months, she lost her weight of 10 lbs not attributable to diet or exercise.

Past Medical History Chronic constipation and hemorrhoids Mild dyspnea on exertion Chronic arthritis in her knees and hands Hypertension controlled using diuretics Mammogram (8 months)-normal Colonoscopy or sigmoidoscopy-no screening  Current Medication: furosemide (100 mg, orally), Ibuprofen (400 mg, orally), oral contraceptives for 10 years   Family history: Mother, 83 years old, has type 2 diabetes and father died of acute myocardial infarction at age 60.
