A 85-year-old female patient has presented with ca. Lung involving ribs, pet scan reveals mediastinal nodes ( not confirmed on bx). No disease elsewhere. bx confirms it to be Sarcomatoid ca. Lung ( IHC CK +, TTF and some other markers -ve).
This is not a very frequent variant to be encountered with and as far as I know surgery remains the main stay of treatment even with N2 disease. The problem is Age Chest wall involvement requiring excision with mesh+mmc reconstruction. So if PFT and cardiac functions allows for surgery (tumour arises from middle lobe not sure whether lobectomy would suffice). Should we proceed with it or anything else can be done? Please share your experiences.
