A 73-year-old man with rheumatoid arthritis presented to the emergency department with chills and a rash on right thigh for two weeks. He was taking prednisone 10 mg daily which was increased to 40 mg daily during frequent exacerbations. Physical examination - Revealed a 6 x 10 cm red, warm patch with 0.5 cm indurated black papules and ulcers (Image A and B).
