She currently takes donepezil 5 mg once daily; a week ago, her family started giving her over-the-counter diphenhydramine for insomnia. She has been constipated for 4 days.

Physical examination reveals an uncooperative patient with a distended urinary bladder. Laboratory studies reveal a serum sodium level of 146 mEq/L, serum creatinine level of 1.5 mg/dL, and a blood urea nitrogen level of 30 mg/dL.
