Pancreatic ductal adenocarcinoma (PDAC) frequently coexists with diabetes mellitus (DM), which is associated with worse prognosis. However, the impact of glycemic control on survival remains unclear. To address this, a study evaluated whether glycemic status influences overall survival (OS) in 255 PDAC patients with DM. Patients with HbA1c < 8% had longer median OS (2.5 years) compared with those with HbA1c ≥ 8% (1.2 years).

Among DM subtypes, long-standing DM was associated with the poorest survival (1.7 years), while type 3c DM patients fared best (7.2 years). In multivariable analysis, poor glycemic control (HR 1.54), age ≥ 65 (HR 1.83), stage IV disease (HR 2.04), and male sex (HR 1.52) independently predicted worse outcomes.
