An 80-year-old female was presented to the ED with angina pectoris and dyspnea. The patient had a history of coronary heart disease, hypertension, and sick sinus syndrome, for which she had received pacemaker implantation. Eight years before the present visit, the pacemaker battery was replaced due to depletion. The patient reported a gap in the follow-up in the last three years.

Laboratory findings Blood pressure: 146/79 mm Hg Heart rate: 65 bpm with a regular rhythm Mild lower limb edema Troponin T: 0.01 ng/ml CK-MB: 1.77 ng/dl NT-proBNP: 2458 pg/ml The electrocardiogram findings revealed right ventricular pacing at 65 beats/min and retrograde P waves. Tricuspid moderate regurgitation and aortic valve calcification were observed with 50% left ventricular ejection fraction.
