Patient history: A 60-year-old female had a severe attack of epistaxis; blood came from both nostrils. She received proper management, and the bleeding stopped. After the removal of the nasal packs, she had a severe frontal headache on the left side, accompanied by a rise in her temperature and a small amount of an offensive nasal discharge. Two days later, the temperature became higher, 40° C; there was impaired consciousness, vomiting, blurring of vision, and some neck retraction and backache.
