A 45-year-old man has been admitted to the postanesthesia care unit. Following surgery to excise an abdominal mass, he was extubated in the operating room. He was stable upon admission to the postanesthesia care unit but rapidly developed a fever and tachycardia. His vital signs at that time were significant for a heart rate of 150 beats per minute, blood pressure of 150/90 mm Hg, respiratory rate of 30 breaths per minute, an oxygen saturation of 92% on 4 L/min oxygen via nasal cannula, and a temperature of 103°F.

He was given acetaminophen, intravenous fluids, and dantrolene. He was started on a non-rebreather mask, packed in ice bags, and transferred to the surgical intensive care unit.
