Case presentation A 35-year-old female presented to the ED with complaints of worsening cyclical pain in the left lower quadrant for the last three days. The patient reported menorrhagia and related the pain to her menstrual cycles. Her periods ended three days before her presentation to the ED. She also complained of nausea and loss of appetite.

Physical examination revealed tenderness to palpation at the left lower quadrant and the presence of a 1 x 3 cm area of chronic purple skin coloration and small nodules. Computed tomography of the patient revealed a soft tissue mass in the left inguinal region with a rim and subcutaneous fluid collected at the inferior anterior region of the mass (Figures A, B, and C).
