Case presentation  A 41-year-old farmer presented with lesions on his right leg (Image 1). He informed that the lesions first appeared 20 years ago on the knee and have been present since then. The lesions had developed and progressed gradually to the dorsum of the foot, resulting in difficulty in walking. Image 1. Lesions on the patient's leg Physical examination  Palpable, coalescent, and subcutaneous nodules with verrucous lesions The lesions were painless and pruritic papules.

Histopathological examination  Examination of a skin scraping using 10% potassium hydroxide revealed the presence of pigmented, thick-walled, and multicellular Medlar bodies (sclerotic cells). A culture of the sample revealed the presence of dematiaceous fungi.
