In various organizations and sectors, human error is one of the leading causes of accidents or mishaps. Medical error is a subtype of human error that refers to any error that occurs during the diagnosis or treatment of an illness and is estimated to cause 44,000 to 98,000 fatalities each year. According to research, medical errors, such as incorrect prescription, maltreatment, and inconsistencies in medical test findings, have been reported among 34% of patients in the United States.
To establish a particular and complete approach for calculating the chance of medication errors, an extensive literature study was done to investigate elements that are thought to influence such errors.
