Case presentation A 14-year-old otherwise healthy female presented to the hospital with lower right jaw pain and intermittent lower lip numbness. The symptoms had begun seven months ago with sharp and radiating right mandibular pain. Medical history The patient was undergoing active orthodontic therapy and initially attributed the jaw pain to her braces. Physical examination The right mandibular incisors and premolars found non-vital. Periapical and panoramic radiographs did not reveal any carious lesions or bony abnormalities.

The oral and maxillofacial surgeon noted bilateral mental nerve paresthesia as the only finding.
