Case presentation A 65-year-old woman presented with fever, worsening fatigue, dry cough, dyspnea, and a weight loss of 25 pounds. The patient was previously diagnosed with myelodysplastic syndrome (MDS), and at the time of presentation, her bone marrow biopsy revealed conversion to acute myeloid leukemia (AML) . Clinical assessment The patient was admitted to the institute and was subjected to induction chemotherapy . During her stay, she developed fever and dyspnea .
She was advised not to drink or eat as she was scheduled for a bronchoscopy. The morning lab values indicated hypernatremia with a sodium level of 154 mEq/L . To further evaluate hypernatremia, the patient was consulted for an endocrinological exam .
