Case presentation A 94-year-old male was presented to the ED with an altered mental status. The patient had a recent medical history of pneumonia, chronic kidney disease, congestive heart failure, hypothyroidism, and myocardial infarction. The patient’s medication included amiodarone, amlodipine, clopidogrel, levothyroxine, lisinopril, metoprolol, propafenone, and simvastatin. The patient was allergic to iodine and had a history of remote use of tobacco.

According to the patient’s caregivers, the patient had become less responsive and increasingly confused over the last 24 hours. Additionally, when the patient was being brought to the hospital, he had a heart rate of 30-40 bpm.
