Case presentation A 65-year-old man presented to the ED with a complaint of intermittent moderate right flank pain over the past few days. The patient took several medications but found no improvement. Physical examination Blood pressure: 140/81 mmHg Heart rate: 78/min Laboratory findings Potassium levels: 4.3 mEq/L Creatinine levels: 0.8 mg/dL Urinalysis revealed trace proteins, white blood casts, eosinophilia, and red blood cell 6/hpf. The patient had a medical history of epilepsy and hypertension. He was receiving valproic acid for epilepsy treatment.
Four months ago, he was switched from valproic acid to lamotrigine due to adverse effects such as thrombocytopenia and hair loss. Six months before the presentation, his physician changed the therapy for hypertension from thiazide to amlodipine.
