A 72-year-old man with a history of hypertension for which he was taking a beta-blocker, an angiotensin-converting enzyme inhibitor, and diltiazem presents to his internist for a routine blood pressure check. His blood pressure was 220/110 mm Hg so he was sent to the emergency department (ED) for acute therapy of hypertension. In the ED, a nitroprusside infusion was started, and he was admitted to the hospital. An ECG was recorded at the ED which is attached with the case description.
