A 17-year-old boy presented to the emergency department with direct trauma to the anterior cervical region. Physical examination The patient had stable vitals. A physical examination revealed the following signs: Laryngeal stridor Hypoventilation Subcutaneous emphysema in the cervical, thoracic, and abdominal regions.
Radiographic examination CT scan revealed disruption of the medial one-third of the trachea (17 mm from the jugular notch) of approximately 5 mm, with the orotracheal tube exiting through it and locating itself anterior to the inferior region of the trachea. The subcutaneous emphysema dissected all the cervicothoracic spaces, continuing to the anterior and middle mediastinum, abdominal wall, and the extraperitoneal and transdiaphragmatic spaces in the bilateral subphrenic spaces.
