A 28-year-old male was admitted to the hospital with an episode of tonic-clonic seizure, left arm paresis, and paraparesis. The patient was confused, drowsy, and experienced complex visual hallucinations. The patient had a fever of 38°C on arrival at the hospital. Medical history The patient experienced disturbance in the gait for two months and was diagnosed with HIV three months ago. Examinations The patient had diminished strength in his left arm (3/5). The lower limbs had symmetric paresis (2/5) along with symmetrically augmented myotatic reflexes.

Babinski’s plantar reflex was absent in the patient. His neurological examination was normal. MRI  of the head was performed to arrive at a conclusive diagnosis (Image).
