A 26-year-old pregnant woman presented to the ER with complaints of severe shortness of breath, sudden onset of palpitations, and coughing. The patient reported experiencing several episodes of palpitations in the past that lasted for a day or two; however, she never experienced dyspnea of this intensity. The patient was 20 weeks pregnant and was taking prenatal vitamin supplements. She reported having a history of rheumatic fever at the age of 14 years   On examination, her heart rate was irregular and fluctuated between 110-130 bpm.

Other vitals were recorded as follows: Blood pressure: 92/65 mmHg Respiratory rate: 24 bpm with labored respiration Oxygen saturation: 94% on room air   Systemic evaluation The patient was producing a scant amount of frothy sputum with a pinkish tint.
