A 39-year-old male bird breeder presented with a five-month history of recurrent skin lesions. Initially, he had painless papules on a number of locations of his body like arms, thighs, chest, feet and hands. Progressively the papules became ulcerated and painful. He had a high temperature and looked pale and anemic. He also complained of weakness and was unable to sit up without support. He had had no previous experience of skin trauma or ulcers.

His blood report showed low red blood cells (RBC) count (2.64 M/mL) and low hemoglobin (7.6 g/dL). WBCs and platelet counts were normal and HIV tests showed negative results. Fluids collected from the bulbous lesions showed negative results for bacteria. Skin biopsy from the ulcers was performed.
