A 65-year diabetic female was evaluated for episodic chest pain (Mild to moderate). NYHA - class II Investigations showed: Serum creatinine- 1.6 GRBS- 210 mg/dl. patient on oral hypoglycemics. ECG- Evidence of old anterolateral infarction. Normal sinus rhythm. X- ray - Normal 2D echo -Ejection fraction: 45%. Mitral regurgitation 3+. All other parameters within normal limits.
Coronary Angio: Triple vessel disease. LAD- 70% Proximal lesion, Nondominant circumflex totally cut off( CTO), OM 3 - Stenosed 60%, PDA 80%. Carotid Doppler- Normal. Intraoperative: Left internal mammary artery harvested - Inadequate flow. Saphenous Veins 3mm, good quality. The surgeon decided to use only veins for grafting diseased coronaries. What is your opinion about the management of this patient?
