Case presentation A 34-year-old female presented to the emergency department with a painful rash on her left foot. The rash first appeared one week prior and rapidly progressed, involving the hallux and second digit, accompanied by erythema and blistering. Physical examination revealed a serpiginous track-like lesion on the affected foot (see cover image). Initial treatment by her primary care physician included valacyclovir, amoxicillin, topical mupirocin, and methylprednisolone.
Despite this, the rash continued to worsen.
