Case presentation A 2-year-old girl presented to the emergency department with a swelling on the left upper eyelid. Patient history The upper eyelid swelling started two days before an episode of vomiting due to a viral illness. There was no associated fever, chills, upper respiratory tract symptoms, lethargy, or behavior changes.
Clinical examination External examination: Mild left-sided ptosis, axial proptosis, upper eyelid fullness with no mass or tenderness, and soft to palpation Visual acuity in each eye: Fix-and-follow Pupils: Round, symmetric, and reactive to light No afferent pupillary defect Full ocular movements Grossly full visual fields to confrontation.
