A 25-year-old male presented with chest pain for the past week. He also complained of blood-containing expectoration of around 300 ml/day for the past two days. History No relevant family history He suffered from shortness of breath and wheeze in his childhood. Investigation Physical examination revealed tachypnea and tachycardia with an oxygen saturation of 96%. Auscultation revealed crackles in the left upper lobe.
Laboratory findings – Hemoglobin: 13.6 gm/dl Total leucocyte counts: 15.500 cells mm3 Erythrocyte sedimentation rate: 44 mm/h Serum IgE: 800 IU/ml The enzyme-linked immunosorbent assay was strongly positive for PR3 autoantibodies.
