Case presented is of a 28 year old male with Systemic lupus erythematosus (SLE) complaining of chronic left hip pain and left knee pain that has not been resolved since one week. Patient is on lisinopril, prednisone, rantidine, hydrochlorothiazide, azathioprine, and diclofenac sodium topical gel. Radiological findings show Sclerosis and density changes and “Crescent Sign”, subcondral radiolucancy (Image 1), Loss of smooth spherical or collapse of femoral head.
Narrowing of joint space, degenerative changes (Image 2), Coronal T1 MRI of both hips shows diffuse low signal throughout the left femoral head, neck, and proximal shaft (Image 3), and Axial T2 MRI of both hips shows diffuse high signal in the left femoral head consistent with the avascular necrosis (Image 4).
