Case presentation A 68-year-old man presented for hemiarthroplasty of the right hip with abdominal swelling. He could not open his bowels or pass flatus for the last four days. Social history Six months ago, he quit smoking History Transitional carcinoma of the bladder Hiatus hernia Underwent appendicectomy Examination Blood pressure - 114/88 mmHg Pulse rate - 98 beats/min Abdomen - Disoriented with mild generalized tenderness and resonant to percussion Absence of hernia Few bowel sounds could be heard Digital rectal examination- Empty rectum Clinical investigations Hemoglobin - 10.2 g/dL White cell count - 12.6 × 109/L Platelets - 422 × 109/L Sodium - 131 mmol/L Potassium - 3.2 mmol/L Urea - 5.7 mmol/L Creatinine - 78 µmol/L An x-ray of the abdomen was performed.
