Case presentation An 80-year-old male with type 2 diabetes presented to his family doctor during a follow-up visit with pruritic, erythematous papular lesions on his trunk and upper limbs (Cover image). The lesions had been progressively worsening over the past six months. He reported generalized pruritus, most notably involving his upper limbs and hands, which did not improve with regular skin hydration. Some lesions evolved into blisters that ruptured and formed crusts.

He denied any association between the onset of symptoms and changes in hygiene products, diet, or exposure to new substances. After moisturizers failed to provide relief, the patient applied an over-the-counter topical methylprednisolone aceponate, which alleviated his symptoms.
