A 68-year-old man presented with the complaint of 9 kg weight loss, persistent right flank pain, and intermittent fever. Medical history – Type 2 diabetes (T2DM) for 10 years, sleep apnea, wearing a CPAP daily Medication history – Canagliflozin and dulaglutide were initiated in the last year but were discontinued and insulin glargine injection was started one week ago. Current medications –  Insulin glargine injection 25 units in the evening and metformin 1000 mg twice daily Physical examination – Random blood sugar (RBS): 193 mg/dL, HbA1c: 11.5% Diagnosis –  T2DM with persistent funguria The patient was referred to a urologist to evaluate funguria. The urologist advised an ultrasound, which revealed right hydroureteronephrosis and mobile debris .
