Case presentation A 23-year-old woman with no prior psychiatric history presented to the ED with disorganized behaviors, bizarre religious delusions, and suicidal ideation, four months after an uncomplicated vaginal delivery. Her mother reported that the patient had shown signs of depression, poor sleep, and withdrawal starting two days after delivery, which progressively worsened over three months. The patient was also grieving the recent death of her father, which appeared to exacerbate her emotional state.
Over time, she developed auditory hallucinations and delusions that “the Devil” was speaking to her. She was observed trying to “purge the Devil” and engaged in dangerous behavior such as breaking glass and removing knives from kitchen drawers.
