History A 68-year-old man was an inpatient in the stroke unit. He recently started a second course of intravenous antibiotics for aspiration pneumonia. He initially improved clinically, and the consolidation on his chest X-rays cleared up. Then he had a high fever and complained of abdominal pain. His nurse reported that he went toilet 8-10 times a day, passing large volumes of greenish liquid stool each time.
Examination The patient’s abdomen was generally tender, and bowel sounds were hyperactive.
