Case presentation A 43-year-old female G2P1 presented at 33 weeks of gestation to the clinic. She had a burning sensation during urination and increased frequency for a week. The patient reported fever and an episode of vomiting. Medical history Diagnosed with diabetes a year before.

Normal, uncomplicated previous pregnancy Physical examination Blood pressure: 110/80 mmHg Heart rate: 104/min Respiratory rate: 16/min Temperature: 102° F Costovertebral tenderness on palpation WBC count: 15,000 cells/mm 3   The diagnostic workup was suggestive of acute pyelonephritis.
