A 24-year-old primigravida (32 weeks of gestation) presented with swelling over both lower limbs and fever for the past one month. The swelling had a symmetrical onset and was progressive with pain and redness. There were no associated complaints like a headache, palpitations, breathlessness, cough, orthopnea, nocturnal dyspnea, jaundice, low urine output, or dysuria. No history of smoking or alcoholism and drug abuse. On examination: The patient was conscious, cooperative, and well oriented but hesitant while speaking and making an eye contact. B.P.: 144/94 mmHg.

Pulse: Sinus tachycardia. Temperature: 102⁰F Fundus and thyroid examination: Normal. Swelling, redness, and tenderness over both lower limbs suggesting cellulitis.
