Mr. K is a 51-year-old gentleman with type 2 diabetes for 10 years, hypertension for 15 years and dyslipidemia for 10 years. He comes to the clinic for a regular follow up. He is asymptomatic. His family history is significant for premature coronary heart disease in his father who died of heart attack at age 43. He is taking metformin 500 mg, twice a day before food and dapagliflozin 10 mg before breakfast for his diabetes, telmisartan 40 mg, after breakfast for his hypertension and rosuvastatin 10 mg after dinner, for his dyslipidemia.
