Case presentation A 58-year-old male patient with small papules on the nape for the past one year presented to the plastic surgery department. The patient had type 2 diabetes mellitus and was treated with oral antibiotics for the papules by his general physician for one month without any improvement. Initial examination Large swelling on the occipital scalp with secondary infection Medical treatment Clindamycin and rifampicin treatment controlled the secondary infection. As per a dermatologist’s suggestion, isotretinoin and dapsone were prescribed, which lead to slight improvement. The patient was willing to undergo a surgical procedure to remove the swelling in the nape.
First stage surgery The entire lesion was excised. Negative-pressure wound therapy (NPWT) was performed. Second stage surgery after one week Split-thickness skin graft (SSG) was performed. NPWT was performed again.…
