Case presentation A 6-month-old female infant was presented with high fever, vomiting, decreased feeding, and irritability. The infant had been previously diagnosed with congenital hydrocephalus and had undergone ventriculoperitoneal (VP) shunt surgery about two weeks ago. Physical examination Temperature - 37.7°C Respiratory rate – 60 breaths/min Heart rate – 136 beats/min CNS examination was done, which showed exaggerated reflexes on all limbs, decreased power, and hypertonia.

However, all other examinations of the pupils, respiratory system, abdomen, and cardiovascular system were normal.
