Case presentation An 18-year-old male volleyball player presented with a three-week history of pain in his lower back. He was otherwise healthy and had no history of previous trauma or injury. However, he stated that the spinal extension with his arms overhead resulted in pain in the lower right back region. Physical examination A normal response to straight leg raise test. The range of motion was normal with extension and flexion of the lumbar spine but was associated with pain over the paravertebral muscles.

Mild tenderness on palpation of the proximal lumbar spine vertebrae was noted. Paraspinal spasms were evident. The stork test revealed a mild pain in the lower back. Radiological investigations No abnormality was detected on the anteroposterior view of the lumbar spine.
