An 82-year-old male presented with burning and itching rashes on his knees. The patient had been suffering from arthralgia and was advised to apply sap from the herb Mandragora radix . He had applied the sap for three days and lesions appeared on the fourth day. The patient was otherwise healthy with a history of gonarthrosis. His knees had erythematous, edematous, scaly lesions. The open application test (OAT) was positive for an allergic reaction to Mandragora radix sap at 48 hours.
