Case presentation A 60-year-old lady reported to the hospital after acute shortness of breath. Her symptoms started two days prior, with progressive deterioration until she had to finally be admitted. The symptoms were forgetfulness, mild fatigue, feeling chilled, difficulty breathing at rest, urinary incontinence, and new-onset and worsening swelling in her bilateral lower extremities. Medical history She had suffered from chronic obstructive pulmonary disorder, a year ago and continues to require bilevel positive air pressure (BiPAP) during her sleep.

Her medical history also includes coronary artery disease, myocardial infarction, hypertension, hyperlipidemia, hypothyroidism, diabetes mellitus, peripheral vascular disease, tobacco usage, and obesity.
