A 75-year-old woman presented to the ENT department with complaints of pain and swelling in right cheek since 3 months. Past history: Recurrent maxillary sinusitis, stage 3 chronic kidney disease, IDDM, IHD, asthma, and previous episode of cardiac arrest. Physical examination: Swelling and erythema in the right maxillary region. No associated diplopia, nasal symptoms, or epistaxis. Neurological assessment was within normal limit. Throat and nasal examination did not reveal any significant pathology.
Investigations: Nasal endoscopy- Large crusty antral opening. Coronal CT scan- Evidence of necrotic bone with marked acute inflammation. Bony destruction in the lateral wall of the right maxillary antrum with bone erosion and thickening. Absent bony dehiscence on the right superior orbital plate.
