Case presentation A 58-year-old female was referred to a pharmacist clinician for pharmacotherapy assessment and diabetes management. She reported frequent feet swelling throughout the day that used to subside while keeping them upright. The patient usually took an extra dose of furosemide to reduce the swelling that prevented her from sleeping due to nocturnal urination.

Medical history The patient had a history of the following conditions: T2DM, hypertension, and hyperlipidemia Asthma that resulted in multiple ER admissions Coronary artery disease Persistent peripheral edema Long-standing history of musculoskeletal pain following a motor vehicle accident Atrial fibrillation with cardioversion Anemia Knee replacement surgery Medications  Current medication for diabetes A premixed formulation of 75% insulin lispro protamine suspension with 25% insulin lispro preparation.
