A 75-year-old woman presented with the confused state. She underwent emergency femoral hernia repair 5 days before. The operation was straightforward and there were no complications from the surgery. Past Medical History (PMH): Osteoarthritis of her right knee, for which she is taking diclofenac. She is a non-smoker and drinks two units of alcohol per week. Physical Examination: She is disorientated with time, place and person. She is pale and tachypnoeic. Blood pressure - 90/70 mmHg.

Pulse rate -110/min. Chest examination-Chest is clear with oxygen saturation of 97 percent on air. On palpation of her abdomen, vague upper abdominal tenderness was noted.
