A 49-year-old woman complained of hearing loss and right facial paralysis. She first noted the hearing loss about eight or nine years ago. Approximately eight months ago, she noticed twitching in the right side of her face, which got progressively worse and then subsided, leaving right facial paralysis. The patient recently developed headaches, vertigo and some visual disturbances. As part of her workup, an MRI brain scan and a temporal bone CT were obtained which revealed a 2.5 cm infiltrative mass in the right temporal bone mastoid area.

The ENT surgeon recommended an excisional biopsy be done to obtain a tissue diagnosis prior to initiating definitive treatment. The patient then underwent a right-complete mastoidectomy.
