Case presentation A 21-year-old man with no relevant past medical history arrived at the emergency department reporting sudden lower throat pain and dysphagia that started immediately after an intense weightlifting session at the gym. He stated that toward the end of his workout, while straining during a maximal lift, he felt a sharp pain in the lower neck and upper chest, followed by discomfort when swallowing.

He denied any neck or chest trauma, recent upper respiratory infection, coughing, vomiting, or foreign body ingestion.
